*I agree to acknowledge that this elective ultrasound service does not replace the medical care of an OBGYN or midwife.

*I agree that if the ultrasound technician at The Baby Suite has a concern about the baby they will suggest they contact their OBGNY provider or Midwife.

*I agree to understanding that the ultrasound technician cannot give out diagnostic information due to this being an elective ultrasound for entertainment purposes only.

*I agree that ultrasound technical error happens and the wrong gender can be given.

*I agree that sometimes babies are in positions where it's difficult to obtain optimal images and I may have to come back for a re-do ultrasound at no extra cost.

*I agree that if I cancel my appointment within 24 hours of my time appointment, I will not be refunded for my deposit.

*I agree that images or videos of my baby can be used for marketing on The Baby Suite's social media pages without the use of my name or any other identifiable information.
